Topic: Understanding the Impact of Human Error

  • 80%-90% of workplace incidents are due to human factors

  • Recurrence of Human Errors is inevitable if the correct root cause is not addressed

  • Re-training is the most common solution used, but is the least effective

  • Learn about the Human Error types and recommended solutions

Veronica Murry: Veronica is a Sr. CAPA and Quality Systems Project Manager at Stryker. She has been with Stryker since July 2000 and has been in various engineering roles from Advanced Manufacturing in R&D, Manufacturing Engineer in Distribution, Process Engineer, and Project Engineer in the Quality Assurance team.   Veronica is Lean Certified for Value Stream Mapping, certified as an Auditor/Lead Auditor for ISO 13485 and an ASQ Certified Quality Engineer. Veronica became a trainer in the Stryker Sentinel program in 2015 which is a continuous improvement program specifically targeted for error prevention and design for human factors.

  

Beverley Walker: Beverley is an Associate Continuous Improvement Manager at Stryker. She has been with Stryker since December 2010. She started her Stryker career as a Training Advisor in Human Resources before moving to Senior CAPA Specialist with the Quality group. During this time, she worked on implementation of an MDI (Managing for Daily Improvement) System. Beverley became a trainer in the Stryker Sentinel program in 2015 which is a continuous improvement program specifically targeted for error prevention and design for human factors. In 2016, she also became the site lead.
